What do I need to bring with me on my first appointment?
On your first appointment, please make sure to review, print and bring the following documents with you on your appointment; Complete Patient Form Packet. In addition, make sure you have your insurance cards, photo id and all bottles of current prescriptions and supplements.

What if I need to cancel an appointment?
We understand that your appointment may need to change. Advance notice, preferably 24 to 48 hours is required to reschedule your appointment. Failure to notify our office may result in a surcharge.
What if I need a prescription re-filled?
Please call your pharmacy first as they send a fax request with all the pertinent information. Refills are authorized 24-48 hours after receiving the request. For chronically used medications, most pharmacies will dispense an emergency supply of 3 days.
We do not call in mail order prescriptions as we have found they often get lost
For those whose pharmacies do not fax, please call our prescription refill line at (386) 423-2550, option 2. You may check at your pharmacy or come to pick up your mail-order prescriptions in 48 hours.
What if I have an emergency after office hours?
If you need immediate attention please call 911. If not, you may call our answering service at (386) 423-2550 and they will notify the on-call physician.
What types of insurance and payments are accepted?
Please call your insurance company to verity our office is covered before scheduling an appointment. In addition, please review our Financial Policy for complete information.
